NEW PATIENT WELCOME PACKAGE

A few days before your first visit to the office you will receive a New Patient Package that includes a biography of Dr. Pickard, Dr. Grewal-Fayetteville, Dr. Grewal-Siloam Springs,  Dr. Mauro or Dr. Rakstang and a welcome letter from them as well. There will also be a Registration Form that you may fill out before you arrive at the office. Doing this before you arrive will make your first visit go more quickly and smoothly.

Also in the pack is a map with directions to the office.  There is also a very informative brochure titled: " A Beautiful Smile Is Always In Style" Finally, a very important part of the package is a discount coupon that you may apply to your diagnostic records fee.  It is good in your first appointed visit only.-
